Council on Aging – Community Needs Assessment

Your voice matters! The Council on Aging of Central Oregon (COA) is conducting a survey to collect public input about the needs of older adults, people with physical disabilities, and unpaid family caregivers in our community. The survey is one part of a larger community needs assessment that the organization is undertaking to help identify gaps, leverage resources, and prioritize services across the tri-county. The data collected in this survey will be used to develop an Area Plan, which is a requirement under the Older Americans Act.
COA especially wants to hear from members of Deschutes, Crook, and Jefferson counties, who are:
- Ages 55+, particularly:
- Black, Indigenous, Tribal Elders, and people of color
- Immigrants and refugees
- LGBTQIA+ older adults and veterans
- People who are homebound or isolated
- Older adults with low income
- Unpaid family and friend caregivers
- Veterans
- Adults with physical disabilities
The survey is available online in English and Spanish through October 15th (Versión en Español: https://forms.gle/oHCiZDV1HekDFHbSA). If individuals prefer to fill out a paper version, or need assistance completing the survey by phone, please call our office at 541-678-5483.
When you have finished the survey, you will be prompted to enter your contact information for entrance into a raffle for a chance to win a $50 Visa gift card, $50 supermarket, or $50 gas gift card. This is optional and your survey data will still remain anonymous.
